I wondered that too, but the nurses hands look fine and the beginning of the word "Hospital" is legible.
If this is 50's or 60's finding a skinny scrawny American wouldn't be too hard.
EDIT: Wait hell I didn't notice the fingers of the nurses were not holding the cigarettes correctly. God damn it the robots tricked me.
Also got tricked at first. The cigar-man is also missing his left arm entirely
skinny yes sure! but unless it's a very tall child in there it'd be close to impossible to get that narrow shoulders
