You should. I've sunk over 200 hours into it. And those are rookie numbers.
Just know that it's buggy, even with the stability mods. RTGame's experience with the crashes in his playthrough is pretty on point.
After getting pegged for 200 hours, you should really take a break (actually probably before that).

Yes, it's a great game, and you start the game by being penitrated (by a bullet to the head)

I'd recommend mods unless you also like being pegged by bugs and crashes. Also possibly something to help with combat, it's a little jank when not using V.A.T.S.
